Step 1:

Schedule a FREE Pre-screening in clinic or Televideo

Step 2:

Submit Payment for $200 Enrollment Fee using the link at the bottom of this page.

​Step 3:

Once Steps 1 & 2 are completed, a chart will be created in our electronic health record system.
You will be contacted about an appointment ASAP.

Step 4:

You will be asked to complete a questionnaire at your appointment. There will be consent forms to be signed and returned. This is a hard stop in the process. Nothing further can legally be done until these are signed and returned.

Step 5: 

Following step 4 your fasting labs will be ordered.
Once we receive them you will be contacted to discuss medication options

Step 6: 

Once program fees are paid, your medication and supplies will be ordered.
